Visits nursing facilities to address acute or chronic conditions when participants are unable to visit the clinic. Conducts home visits for participants who are homebound due to acute or chronic health concerns. Completes initial assessments for all new participants and conducts quarterly evaluations throughout their first year of enrollment. Limited Local Travel Oversee Care and Treatment Plans Collaborate as part of the Interdisciplinary Team Supervise and Mentor Clinical Staff Participate in Phone-Only Call Rotation (1 in 15 weeks) See 1015 patients per day Licensed Nurse Practitioner in the state of Massachusetts Certified as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) 1 Year of Recent Experience as a Nurse Practitioner is Required Experience in primary care or geriatrics preferred Current DEA registration in Massachusetts Current CPR Certification COVID-19 Vaccination Required or Exemption

The Position
The Nurse Practitioner is an essential member of the Interdisciplinary Team contributing to the development implementation and evaluation of individualized care plans aligned with the model. They are currently seeking a Nurse Practitioner to join their established team in Brighton MA providing care to patients in both the clinic and in their home or in a facility.
Salary and Benefits
- Salary Range : $130000- $145000 Based on Experience
- Health Insurance : Employer pays 75% of premium
- Vision and Dental Insurance
- 26 Days Combined Paid Time Off :
15 Vacation Days
