What You'll Do

• Identify and engage hard-to-reach patients, addressing health and social needs through trust-building and support.
• Connect patients to healthcare, community resources, and benefit programs while reducing barriers to care.
• Collaborate with providers, case managers, and agencies to coordinate services and ensure smooth care transitions.
• Coach patients toward health goals and assist with appointments, applications, transportation, and home visits.
• Advocate for vulnerable populations while maintaining flexibility, cultural sensitivity, and continuous learning.
