A company is looking for a Disease Management Nurse-Full-Time.

Key Responsibilities

Coordinate member outreach for health promotion and restoration

Create and implement individualized care plans for patients

Track high-risk member groups and monitor health improvement outcomes

Required Qualifications

Graduation from an accredited RN program

Current United States RN license

Prior experience in case management or wellness program coordination

Proficiency in Microsoft Office Suite and claims processing tools

Understanding of national care guidelines for chronic disease management
