A leading acute-care hospital in Fort Wayne, Indiana is seeking an experienced Travel RN – Care Management for a day-shift assignment focused on discharge planning and care coordination. This is an auto-offer opportunity for well-qualified candidates with strong hospital-based care management experience.​

Position details

  • Title : Travel Registered Nurse – Care Management
  • Location : Fort Wayne, Indiana​
  • Shift : 8 : 00 AM – 4 : 30 PM, holidays required if they fall during the contract
  • Guaranteed hours : 36 per week
  • Duration : 13 weeks
  • Estimated traveler pay : $60.00 hourly

    • Requirements

  • Active RN license (compact-eligible)
  • Minimum 4 years of RN experience with hospital-based discharge planning / care management required​
  • Proven ability to manage a caseload of approximately 1 : 20 patients , coordinating safe and timely discharges and transitions of care​
  • Proficiency with Cerner EMR for documentation and interdisciplinary communication​

    • Responsibilities

  • Lead and coordinate discharge planning, ensuring safe transitions to home, SNF, rehab, or other levels of care while addressing barriers and resource needs.​
  • Collaborate with physicians, bedside nurses, therapists, social work, and external partners to develop and execute individualized care plans for a high-volume caseload.​
  • Perform utilization and care management functions as assigned, document in Cerner, and communicate clearly with patients and families regarding discharge plans and follow-up.​

  • Typical patient ratio : 1 : 20 care management caseload
  • Department attire : Black scrubs or business casual
  • EMR : Cerner ​

    • This role is ideal for a seasoned RN care manager who thrives in a fast-paced acute-care environment, is comfortable with a large caseload, and has a strong background in hospital discharge planning and coordination.
