Position Summary :

The Patient Care Coordinator - Hospice is a high-level clerical position. This position requires the ability to coordinate care within the hospice program. The Patient Care Coordinator - Hospice may be requested to coordinate activities between inside / outside clinical resources. The Patient Care Coordinator - Hospice will report directly to the Clinical Manager. This position will ensure our organizational quality goals are met, and care remains compliant with all relative regulations, policies, and procedures. Day-to-day activities require assistance with scheduling changes, documentation of information, obtaining orders, updating medications, and assisting in the management of supplies, items and services related to the hospice benefit and the plan of care.

This position entails standing for long periods of time. While performing the duties of this job, there will be considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and / or transferring of a 100-pound patient. Must possess sight / hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must have a current valid driver’s license, auto liability insurance and reliable transportation as travel may be required.

The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. Occasional travel to different locations may be required.
