Position Overview :

The role of a Case Planner, I will participate in training for and provide specialized case management using the family systems based  FFT-CW model for a case load of 10-12 families. The goal of a Case Planner is to reduce risk while increasing protective factors for the families in order to prevent the out of home placement of children.
