Additional Info : Offer high-quality health care for our participants with an entire team of clinicians and other professionals with experience in the needs and concerns of older adults

You will participate as a member of the PACE Interdisciplinary Team in the care of high-risk older adults living in the community with a strong focus on the participants' goals as well as excellent geriatric care management.

FLEXIBLE SCHEDULING : Monday-Friday scheduling, with a possibility for a 32-hour workweek or 40-hour work-week with 1 day remote. To be discussed during the interview process.

QUALIFICATIONS

Must be comfortable with the aging and elderly patient population

Quickly recognize situations / problems before becoming acute, implement effective solutions, and understand consequences

Board certified in IM or FM. Board certification in Geriatric Medicine is PREFERRED

Bilingual in Spanish highly regarded!

Current MA license, DEA prescription privileges, and CPR Certification
