Hourly Pay Range :

$38.16 - $59.15 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights :

• Position : Registered Nurse Case Manager
• Location : Elmhurst Hospital
• Full Time : 36 hours per week
• Hours : Monday-Friday, 8am-4 : 30pm - will work 5 days one week and 4 days the other week per pay period - rotating weekends and holidays required
• Travel : May travel to Edward Hospital (Naperville, IL) for coverage as needed

Summary :

Provides case management for patients in the acute care setting. In collaboration with the physician and other health care team members, the RN Case Manager coordinates the patient's care to achieve optimal patient outcomes.
