Alaris Health is currently looking for a qualified professional with nursing home social service experience to fill an opening for a Social Worker with a privately owned long-term care facility.

Responsibilities :

Assist with planning and implementing a comprehensive social services program which provides for counseling and other support services for residents and families in the facility;

Identifies medical-related social needs of residents, provides appropriate services to meet the individual, as well as collective needs of residents, maintains record relating to the residents' social work needs and care;

Works directly with residents and families' concerns related to the resident's physical and emotional condition;

Evaluates each resident's social needs then formulates plan for providing care;

Acts as a liaison between residents, families, outside agencies, and the facility to ensure that the resident's rights are maintained

Qualifications :

Bachelor's degree in Social Work

Must have excellent communication skills

Proven leadership and managerial skills
