Description :
The Physical Therapist partners with residents and other care providers to provide physical therapy services to maximize residents' functional abilities and independence in their activities of daily living.
The therapist performs evaluative and therapeutic procedures under the order of a physician or other accepted healthcare provider.
Evaluates residents per department protocols and establishes a therapeutic plan of care which includes functional long- and short-term goals.
Seeks input from the resident and primary caregivers regarding the therapeutic plan of care and incorporates this information into the therapy goals.
Provides consultation to the referring physician and other members of the healthcare team regarding the plan of care and discharge needs of the resident.
Attends pertinent team meetings to enhance collaborative care.
Monitors and evaluates treatment effectiveness and modifies the treatment program as necessary to progress toward established functional goals.
Documents all evaluative and therapeutic interactions with the resident per department procedures in the Electronic Medical Record.
Ensures documentation is accessible and understandable to all members of the care team.
Recommends and obtains appropriate assistive devices for residents and provides instructions for their use.
Educates residents and / or caregivers, documenting the results of that teaching.
Refers residents appropriately to Physical Therapist Assistants when necessary.
Collaborates with other therapy professionals to ensure quality resident care.
Understands and complies with reimbursement regulations, documenting care appropriately according to Medicare, Medicaid, and other requirements.
Provides in-service education regarding clinical knowledge and skills to nursing and other departments.
Assists with the development of new programs and services offered by Rehabilitation.
Maintains awareness of professional issues through membership in professional organizations and professional publications.
Supervises the activities of assistant-level staff when appropriate, considering their education and training levels.
Ensures adherence to all state mandates and professional standards for using assistant-level staff.
Provides mentorship to new staff during orientation, ensuring completion of departmental orientation checklists and basic competencies.
Maintains cleanliness and organization in office and clinical areas, assisting with equipment and space cleaning.
Follows all infection control procedures.
Maintains required licenses, certifications, and competencies, completing annual compliance courses on time.
Performs other duties consistent with this role as needed.
