Care Coordinator (I / II / III)

Department : Patient Access & Contact Center

About the Role

The Care Coordinator is a vital member of the Patient Access & Contact Center team. You’ll support high-quality, accessible specialty care by handling inbound communications, appointment scheduling, insurance verification, and related administrative tasks—delivering a seamless, responsive patient experience.

Key Responsibilities

Care Coordinator I

Answer incoming calls, voicemails, and messages regarding appointment requests, medical records inquiries, and telehealth access issues.
Triage and escalate calls to the appropriate subject-matter expert as needed.
Schedule new and follow-up patient appointments.
Verify insurance and collect applicable copays prior to or at the time of service.
Monitor and respond to inbound faxes, SimpleChime messages, and AdvancedMD (AMD) communications.
Route urgent calls and escalations appropriately.
Monitor inbound Zoom Contact Center (ZCC) queue activity and escalate issues when needed.
Track and report ZCC metrics and performance indicators.
Process and manage faxed patient information securely and accurately.
SCOA-based staff only : Check patients in and collect copays during in-person visits at the Specialty Clinic of Austin.
Perform other duties as assigned.

Care Coordinator II

All Care Coordinator I responsibilities, plus :

Serve as a liaison for training new team members.

Assist with problem solving and resolving patient concerns.

Care Coordinator III

All Care Coordinator I & II responsibilities, plus :

Lead and be accountable for new-hire training and development for 30-, 60-, and 90-day expectations.

Support Clinical Operations Managers and Directors in various responsibilities.

Serve as a team lead to disseminate updates on a routine basis.

Foster overall team productivity.

Set team goals for daily, weekly, and monthly objectives.

Qualifications

Skills

Bilingual (English / Spanish) a plus.

Knowledge of medical terminology preferred.

Strong verbal communication; friendly, professional phone etiquette.

Quick and accurate data entry.

Ability to stay calm under pressure.

Basic understanding of insurance and scheduling.

Comfort using phone and messaging systems.

High attention to detail.

Experience

Care Coordinator I

Experience in an office environment using the telephone and computer as primary tools.

Previous EMR / EHR experience a plus.

Telemedicine experience a plus.

Care Coordinator II

Meets Care Coordinator I requirements.

Minimum 12 months of experience at TeleMed2U or equivalent.

Proficient across all areas of the EMR / EHR; able to navigate seamlessly and complete most, if not all, functions.

Demonstrates cross-functional aptitude across most areas of the division.

Care Coordinator III

Meets Care Coordinator I & II requirements.

Minimum 24 months of experience at TeleMed2U or equivalent.

Expert in all systems, tools, and EMR / EHR functionality, as applicable.

Education

High school diploma or GED required.

Physical Requirements

Prolonged periods of desk work and computer use.

Ability to lift up to 15 pounds.
