Geriatrics Nurse Practitioner

Details :

• Setting : In-home must be willing to go in patients homes at least 3 days a week
• Location : Erie, PA (Zip 16501)
• Schedule : Mon-Fri (8a-5p includes travel time for in-home visits)
• Duration : 6 months open to extension or locums to perm
• Duties :
• APP's serve multiple serious conditions like CKD Chronic Kidney Disease and / or ESRD / ESKD (end stage renal disease / kidney disease)
• Conducts assessments including annual wellness exams
• Counsels and educates patients and their families about programs
• Responsible for coordination of care with primary care providers
• Part of the Integrated care team working with RN and Social Worker
• Prescribe medications, order tests, collaborate with Monogram physician
• Follow up and evaluate patient's status
