Telehealth Case Manager RN

A company is looking for a Case Manager - Registered Nurse - Telehealth.

Key Responsibilities

Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs

Utilize clinical tools to conduct evaluations and apply clinical judgment to address health and social indicators

Collaborate with supervisors and stakeholders to overcome barriers and enhance member wellness

Required Qualifications

Registered Nurse with an unrestricted nursing license and multi-state / compact privileges

3+ years of clinical / nursing practice experience

1+ year of medical case management, care coordination, or discharge planning experience

High-speed internet access and proficiency in MS Office Suite applications

Associate's degree in nursing required; BSN preferred
