[job_card.job_description]LOCUM Medical Director role Monday-Friday, full-time No weekend requirements listed On-call rotation required (details TBD) Interdisciplinary PACE care model Leadership responsibility over PCPs, APPs, and clinical workflows Focus on chronic disease management and community-based elder care Perform histories, physicals, and periodic medical assessments Develop individualized treatment plans Participate in applicant evaluations for program eligibility Communicate prognosis and care goals with participants and caregivers Attend interdisciplinary team meetings and care conferences Educate participants and families on chronic disease and wellness Collaborate closely with nursing, therapy, and social work teams Supervise PCPs and APPs Order and interpret labs, diagnostics, and document appropriately Participate in provider on-call rotation Support quality improvement and operational goals Strong assessment and chronic disease management skills Experience working with frail / elderly adult populations Comfort working in a collaborative, interdisciplinary setting Ability to manage dynamic participant needs Active Massachusetts state medical license - Required Board Certified in Internal Medicine or Family Medicine - Required Board Certification in Geriatrics - Preferred DEA registration - Required Prior supervisory experience - Required Minimum 1 year geriatric or frail-elderly clinical experience - Required

This role combines medical oversight, interdisciplinary collaboration, and person-centered geriatric care.
