Role Description

Responsibilities :

• Implement the Plan of Care prescribed by the supervising physical therapist, asjusting as needed based on patient response.
• Employ exercise, manual therapy, therapeutic activities, neuromuscular re-education and or modalities.
• Monitor existing Plan of Care as it relates to established functional and objective goals.
• Regularly consult with and discuss patient status and progress with supervising PT.
• Instruct patient in the use of DME supplies.
• Develop and educate patient / family regarding HEP's.
• Assist in the collection of objective measures and functional outcome measures.
• Maintain complete and accurate documentation per visit regarding treatment tolerance, progression, and subjective reports.
• Participate in case conferences, clinical reviews
• Communicates with agencies / team members and supervisors as appropriate.
• Maintain patient rights and confidentiality.
• Participate in new employee mentorship program.
• Participate in performance during first 90 days and once a year thereafter.
• Maintain accurate and up-to-date personnel requirements.
• Participate company inservices and quality improvement initiatives
