Role Description

Responsibilities :
- Implement the Plan of Care prescribed by the supervising physical therapist, asjusting as needed based on patient response.
- Employ exercise, manual therapy, therapeutic activities, neuromuscular re-education and or modalities.
- Monitor existing Plan of Care as it relates to established functional and objective goals.
- Regularly consult with and discuss patient status and progress with supervising PT.
- Instruct patient in the use of DME supplies.
- Develop and educate patient / family regarding HEP's.
- Assist in the collection of objective measures and functional outcome measures.
- Maintain complete and accurate documentation per visit regarding treatment tolerance, progression, and subjective reports.
- Participate in case conferences, clinical reviews
- Communicates with agencies / team members and supervisors as appropriate.
- Maintain patient rights and confidentiality.
- Participate in new employee mentorship program.
- Participate in performance during first 90 days and once a year thereafter.
- Maintain accurate and up-to-date personnel requirements.
- Participate company inservices and quality improvement initiatives
