POSITION SUMMARY - RN Case Manager

POSITION DUTIES - Completing initial assessments, DC planning (including nursing homes, home health, DME, prior authorizations, etc) in an inpatient setting.

MINIMUM REQUIRED QUALIFICATIONS - CA RN license

BLS

3 years' nursing experience in an acute hospital setting

Prior care coordination experience in a CommonSpirit / Dignity / CHI hospital

LENGTH OF ASSIGNMENT - 13 weeks

SHIFT / HOURS PER WEEK - 8hr days

SYSTEMS - Cerner

START DATE - ASAP
