(FT / PT) The Memory Care Coordinator oversees the overall implementation, delivery and coordination of the Connections Memory Care neighborhood resident services, including engagement and supports the HWD in all health services. Coordinates engagement aspects of life for Memory Care residents including participating in the development of personalized plan or program for successful aging in community. Ensures residents satisfaction and achievement of their realistic goals. Motivates residents to function at highest level of independence.
STARTING WAGE : $50,000 / Year
SHIFT : Monday - Friday 8am-5pm Valid
Licensed Practical Nursing preferred with 3 years of Nursing experience including management.
