A company is looking for a Behavioral Health Care Manager to support the mental health needs of individuals affected by neurodegenerative diseases.
Key Responsibilities
Provide care coordination and behavioral health support within the Collaborative Care Model (CoCM) program
Screen and assess patients for mental health disorders, facilitating engagement and follow-up care
Collaborate with medical providers and develop treatment plans for patients, monitoring clinical symptoms and outcomes
Required Qualifications
Active LCSW (or similar) licensure
Minimum of 2 years' experience in direct case or care management
Background in neurodegenerative or chronic disease care, such as ALS or Parkinson's Disease
Familiarity with insurance processes and medical billing / coding
Knowledge of current trends in neurodegenerative diseases and clinical trials
