Your Impact as a Transplant Coordinator : As a Registered Nurse Transplant Coordinator, you will be essential in caring for patients with chronic organ failure and end-stage organ disease. Your role involves coordinating care to ensure optimal patient outcomes through effective communication with healthcare teams and patients.
Key Responsibilities :
Create and update comprehensive care plans tailored to patients' educational, financial, and discharge needs while maintaining accurate documentation.Engage with the Transplant / VAD team, patients, families, and physicians to facilitate clear communication.Provide education for patients, families, and the community about VAD and transplant processes.Contribute to the development and revision of policies for the transplant program.Commit to professional growth through continuous education and networking.Coordinate patient testing for transplant evaluation and maintain accurate UNOS documentation.Qualifications :
