About the Role : As a Nurse Practitioner with Matrix, you will conduct comprehensive health assessments during in-home visits that can last up to one hour. These assessments include evaluating medical history, medication adherence, and social environments to gain unmatched insight into overall health and well-being. Your responsibilities will include :

• Performing adult and geriatric assessments, including medical history and treatment planning, while following established Matrix protocols.
• Administering and ordering necessary point of care and lab-based tests during assessments.
• Collaborating with physicians, case managers, social workers, and family members to ensure holistic patient care.
• Working closely with Primary Care Physicians to provide education and follow-up based on assessment findings.
• Offering services in the member's residence, including homes and skilled nursing facilities.
