This role is ideal for RNs who thrive in autonomous environments and enjoy community-based patient care. As a Home Health RN, you will assess patient needs, create individualized care plans, deliver in-home clinical care, coordinate referrals, and collaborate closely with physicians and interdisciplinary team members. Key Responsibilities Conduct patient assessments to determine eligibility and care needs Develop & update patient-centered plans of care Provide culturally sensitive, holistic nursing care in the home setting Review medications for safety, effectiveness, interactions, and compliance Document care accurately in EMR (Meditech) Perform skilled nursing procedures per physician orders Educate patients & families on disease management, safety, and emergency plans Coordinate multidisciplinary referrals and discharge planning Conduct Home Health Aide supervisory visits every 14 days Maintain productivity and meet documentation standards Participate in QA, case conferencing, and performance improvement initiatives Travel to patient homes including areas such as Skiatook and surrounding communities Required Qualifications Valid Oklahoma RN License (or multistate equivalent) Graduate of an accredited School of Nursing Valid driver's license, clean MVR, and personal vehicle with minimum liability coverage Reliable transportation for home visits Proficient computer skills Strong clinical judgment, communication, and interpersonal skills Home Health experience required About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
