Job Requirements

Education Requirements : Bachelor’s degree in Physical Therapy from an accredited institution\n\tCertifications Requirements : NBCOT certification, Montana PT license, BLS certification\n\tExperience Requirements : Minimum of 1 year of PT experience, preferably in SNF settings; experience with PDPM and Section GG preferred\n\tOther Requirements : Familiarity with NetHealth / PointClickCare EMR system; strong interpersonal skills; ability to work independently and manage time effectively\n\n\n

Job Responsibilities

Provide skilled physical therapy assessments and develop individualized treatment plans for patients in a rehab setting.\n\tCollaborate with healthcare team members to coordinate patient care and ensure the achievement of therapy goals.\n\tMaintain accurate documentation of patient progress and therapy sessions using NetHealth / PointClickCare EMR.\n\tExecute therapy interventions and modify treatment plans based on patient response and progress.\n\tEducate patients and their families about post-treatment care, exercises, and injury prevention strategies.\n\n\n
