Community Resource Navigator

A company is looking for a Community Resource Navigator, Remote (PA / NJ / DE).

Key Responsibilities :

Conduct outreach to members with potential SDOH-related needs and perform psychosocial assessments

Educate members and families on available benefits, programs, and community resources

Coordinate referrals to internal programs and external agencies

Qualifications :

Bachelor's degree, preferably in social work, psychology, or human services

Minimum of 3-5 years of experience addressing social determinants of health and working with vulnerable populations

Proficiency in documentation and use of care management systems

Knowledge of community resources and social service systems
