United Health Care Services is seeking a reliable and compassionate Transportation Provider to join our State Medicaid Supported Community Connection (SCC) program. The primary responsibility of this role is to transport program participants to and from various community activities, appointments, and services. The SCC program supports individuals with disabilities in engaging with their communities and improving their quality of life.

Key Responsibilities :

• Safely transport SCC participants to scheduled activities, appointments, and other destinations.
• Assist participants in and out of the vehicle as needed.
• Ensure the safety and comfort of all passengers during transport.
• Maintain a clean and well-functioning vehicle.
• Communicate effectively with participants, caregivers, and program staff regarding transportation schedules and any potential delays or changes.
• Keep accurate transportation logs and records in compliance with Medicaid requirements.
• Respond to any transportation-related emergencies or issues in a calm and professional manner.
• Mitigate and manage extreme behaviors during transportation and SCC activities in accordance with program protocols, ensuring the safety and well-being of all participants.
