Clinician - Adult Crisis Stabilization
Adult Crisis Stabilization helps prevent emergency room visits and assists people as they step down from inpatient hospitalizations. Last year we served 141 people who otherwise would have had to visit the ER for mental health reasons. Working closely with the new Mental health Urgent Care and other community supports, Adult Crisis Stabilization is a critical part of Vermont's system of care.
The Clinician provides screening, assessment, diagnosis, treatment, care coordination and discharge planning to increase support and overall wellbeing, improve functioning, and decrease symptomology. This role develops, implements and coordinates treatment plans as part of a multidisciplinary team. Services may be program-based, provided via telehealth, co-located with another agency and / or in the field including home-visiting.
