This position is based in Lakeland, serving patients in Davenport, Haines City, Kissimmee.

Your Role as a Licensed Practical Nurse (LPN) :

• Deliver high-quality nursing care to patients in their homes based on individualized care plans.
• Perform a variety of tasks, including :

Acute and chronic disease management.
