We are currently seeking a Registered Nurse Case Manager to join our rapidly growing team to oversee Nassau County, Suffolk County and Queens County.  At Affirmed Home Care, we believe in compromising nothing when it comes to providing exceptional, client-centered care. This philosophy extends not only to our clients, but to our staff as well.

As the RN Case Manager, you will be responsible for developing and maintaining client-specific care plans, ensuring these plans are followed, and providing for the client’s ongoing needs in a private pay, concierge level home care. This role requires  70% fieldwork  for a small caseload of clients demanding great attention to detail.

The salary for this full-time position ranges from $110,000 to $130,000, commensurate with related experience.

Qualifications :

• Current NYS RN license in good standing
• BSN preferred
• Minimum of two years’ recent clinical experience within a Home Care setting
• Recent hands-on experience with advanced clinical skills including ventilator management, tracheostomy care, G-tube / PEG tube maintenance, IV infusions, and wound care
• Valid driver’s license and access to a reliable car
• Strong assessment and organizational skills
• Strong time management skills
• Prior supervisory experience with field staff
• Proven track record of working independently
