Day Center CNA
The Day Center CNA (Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care.
Responsibilities include :
- Following the care plan and assisting participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting.
- Documenting care provided in accordance with established policies, guidelines, and applicable regulations.
- Carrying out, reporting, and documenting care given in an effective, timely manner as observed by the RN and through periodic record reviews.
- Assisting participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed.
- Monitoring participants' health status such as vital signs, weights, skin condition, appetite and changes in functional ability and reporting pertinent information to clinical supervisor in a timely manner.
- Collaborating with multidisciplinary teams in care planning for participants.
- Monitoring appetite and looking for changes in functional ability and skin condition.
- Monitoring / assisting participants in a courteous manner with meals and helping with feeding, as necessary.
- Reporting any variance, accident, or unusual occurrence to the appropriate personnel.
- Assisting with loading and unloading of participants from vehicles.
- Assisting the transportation department with participants' take-home supplies.
- Serving as a bus attendant on company vehicles while InnovAge drivers transport participants to / from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities.
- May rotate into In-Home Services as a Homecare CNA as deemed necessary by the Center Manager.
- Creating and participating in group(s) and one-on-one activities in accordance with daily activity calendar when needed.
- Attending in-service and continuing education programs as scheduled and necessary.
- Other duties as assigned.
Required :
Active and valid state-issued Certified Nursing Assistant (CNA) license.Current CPR certification or the ability to obtain CPR certification.High School Diploma or GED equivalent.Preferred :
