Primary Care Advanced Practice Clinician Opportunity with Samaritan Health Services – Lebanon, Oregon

Samaritan Health Services, a non-profit integrated healthcare delivery system, is seeking a physician associate or nurse practitioner to join our Family Medicine team in Lebanon, Oregon. Samaritan Family Medicine-Medical Plaza is an established certified Rural Health Clinic with experienced clinicians who are passionate about building healthier communities within a certified Patient Centered Medical Home model of care.

Job Specifics

• 1.0 FTE (34 hours per week) working 4.5 days a week
• Call schedule : phone call only with 1-2 days a month plus one weekend every 2-3 months
• Templated schedule of 40 / 30 or 30 / 15 minute visits
