After-Hours Hospice Nurse

Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. The After-Hours Nurse provides skilled nursing and hospice care to maximize the comfort and health of patients.

MUST BE BILINGUAL IN CANTONESE

Schedule :

Tuesday, Wednesday & Thursday : 4 : 00pm - 12 : 30am

Saturday & Sunday; 2 : 00pm - 10 : 30pm

Location :

San Francisco County and Marin County as needed

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to) :

• Provide continuing care to patients / families as requested and in compliance with agency policies and procedures and applicable federal and state regulations
• Under the guidance of the After-Hours Nurse, responsible for patients / families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care
• Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient / family
• Maintain regular communication with Team Leaders regarding patient / family status and unusual or potentially problematic patient / family issues
• Attend patient care conferences as requested

$54.00 - $66.20 hourly + $12.50 / hr. differential + $1.92 per hour language differential
