Nurse Practitioner

Job Summary

Our client is seeking a dedicated Nurse Practitioner to join their team in providing exceptional care to the senior population. The position involves building strong relationships with patients within a Value Based Care Model, working alongside a doctor, and addressing non-complex cases.

Key Responsibilities

• Prescribing medicine and managing non-complex cases.
• Following a full schedule including an 8 am team huddle, patient consultations, lunch breaks, and administrative duties.
• Engaging in a Value Based Care approach that aligns with Medicare Advantage.
• Ensuring extended patient appointments (45 minutes each).

Qualifications

• BLS or ACLS Certification required.
• NP License in state or in process.
• Family NP background instead of hospital NP.
• Several years of Primary Care experience, including with Geriatrics, or VBC experience.

Experience

• 2-3 years of experience in Nursing Practice.
• Experience working with the elderly patient population.
• Primary Care experience with an emphasis on geriatrics.

Skills

• Proficiency with the EMR system, eClinical Works.
• Strong interpersonal skills for patient relationship building.
• Experience working within a Medicare Advantage Value Based Model is advantageous.

Additional Requirements

• Monday to Friday, 8 am to 5 pm working hours.
