The Home Therapy Registered Nurse plays a crucial role in empowering patients who choose home dialysis. This position is dedicated to providing exceptional care and support to all patients, ensuring they receive the highest level of service in every interaction.
- Training Patients : Instruct patients on home hemodialysis or peritoneal dialysis techniques both in-center and in their homes, ensuring compliance with regulations.

- Work Schedule : Enjoy a Monday to Friday schedule from 8 am to 5 pm, with the possibility of modified hours based on patient needs.

Essential Duties :
Understand and comply with relevant laws and regulations regarding home dialysis.Train patients and care partners in self-care for Peritoneal Dialysis or Hemodialysis as per established standards.Conduct home visits to evaluate the patient’s environment and provide ongoing assessments to enhance care quality.Communicate patient assessments and progress with the healthcare team to ensure tailored patient care.Document all patient interactions in the Electronic Medical Record accurately to ensure continuity of care.Engage in infection control practices and participate in quality initiatives.Be prepared for emergencies by reviewing procedures with patients and ensuring they are familiar with necessary protocols.Manage clinic and patient supply inventories to meet patient needs timely.Use analytical skills to pursue continuously improved patient outcomes and participate in quality improvement data collection.Conduct lab tests as directed and communicate critical results promptly.Review patient medications monthly and administer as prescribed.Be available for on-call duties as scheduled.Demonstrate flexibility in staffing and location as needed to support patient care.Maintain regular attendance as required for this role.Specialization in Home Hemodialysis :
Coordinate necessary evaluations with technical services to ensure home setup meets operational standards.Ensure compliance with water sampling and report results as necessary.Qualifications :
Active RN license in the applicable state, maintained in good standing.At least 12 months of nursing experience, with 3 months in the specific home dialysis modality.Preferred experience with patients on maintenance dialysis.Current CPR certification required before patient assignments.Ability to distinguish all primary colors.Successful completion of the Home Therapy Nurse Orientation program is mandatory.Strong communication skills in English, both written and verbal.Basic computer skills necessary, with proficiency in clinical applications required within 90 days of hire.Meet any additional practice requirements applicable in the state.Capable of passing background checks and any additional requirements for the assigned program.U.S. Renal Care is proud to be an Equal Opportunity Employer, welcoming individuals with disabilities and veterans.
