The Pharmacist Clinical Specialist (Critical Care) is responsible for the provision of pharmacotherapy services for critically ill patients. The Pharmacist Clinical Specialist (Critical Care) will facilitate the development, implementation, and maintenance of systems that promote optimal safety, desirable patient outcomes and pharmacotherapeutic treatment for critically ill patients. This position facilitates pharmaceutical care services, direct patient care programs, medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards. Clinical areas of service line focus include the critical care unit, cardiovascular laboratory, and interventional radiology.

Responsibilities

• Actively participate in rounds as a member of the interdisciplinary critical care team to provide comprehensive medication management for all critical care patients.
• Completes critical patient monitoring and reviews patient profile / chart
• Provides pertinent, timely, comprehensive drug information to the critical care, cardiovascular and interventional radiology teams
• Assists with medication reconciliation for critical care patients at the time of critical care admission, transfer from the critical care unit, or discharge to home or another facility.

Salary Range

The expected base rate for this Glens Falls, New York, United States-based position is $60.00 to $85.00 per hour. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
