Position Overview :

We are looking for a dynamic and autonomous Nurse Practitioner. The successful candidate will play a crucial role in providing direct patient care, developing chronic care plans, and collaborating with referring providers to optimize patient outcomes.

Responsibilities :

• Clinical Leadership :

Provide direct patient care through all available channels and products.

Develop personalized chronic care plans for patients with wounds, focusing on long-term management and prevention of complications.

Establish and maintain effective communication channels with referring providers to ensure continuity of care.
