Description :
As an integral part of a team, the Virtual Care Coordinator is responsible for supporting virtual patient care activities and promoting patient-focused care.
This role involves performing various remote monitoring duties related to patient safety and clinical needs.
Responsibilities include providing support and assistance with telemedicine services and monitoring the hospital environmental status.
The Virtual Care Coordinator collaborates on implementing planned telehealth and remote patient monitoring programs.
Collaboration with leadership to coordinate training and participate in the development of telehealth best practices is essential.
Clear communication and collaboration with the clinical team, patients, technology partners, engineering, and vendors are required.
The role shares accountability for clinical quality and safety outcomes.
Essential Duties and Responsibilities :
Perform quality assurance and peer reviews, evaluate telehealth and virtual patient observation programs in collaboration with leadership, and make recommendations for improvement.
Meet with senior leadership regularly to discuss quality metrics and provide feedback on workflow expansion.
Collaborate and lead case reviews and implement solutions, monitoring outcomes.
Triages and assigns cases for virtual monitoring and observation.
Work with leadership on telehealth policy and workflow development and implementation.
Run and maintain an operational database including relevant program performance, response times, and quality metrics.
Identify concerning trends and participate in developing and implementing corrective action plans.
Assist staff in efficiently utilizing telehealth platforms and technology to meet organizational goals.
Complete Virtual Care Coordinator orientation successfully.
Provide training on telehealth workflows, platform applications, and documentation requirements to new staff.
Serve as a liaison between clinical staff and technology partners on issues related to telehealth platforms and workflows.
Other Duties :
Facilitate camera utilization by analyzing virtual patient care utilization reports, monitoring real-time data, and assessing eligibility for virtual care.
Coordinate transitions with hospital care teams based on patient criteria reports.
Navigate multiple technology platforms and work with healthcare providers to ensure quality care and optimal outcomes.
Provide real-time support to troubleshoot issues related to telehealth platforms and equipment.
Perform continuous observation and surveillance of assigned patients to monitor changes in clinical status or behavior.
Conduct clinical early warning sign monitoring such as vital signs and other health indicators.
Demonstrate interpersonal skills to effectively communicate and collaborate with patients, visitors, and clinical team members.
Report any variations or concerns in a patient’s clinical status and behavior to the clinical team immediately.
Document patient behavior and interventions and escalate to the clinical team when necessary.
Monitor telehealth consult orders and scheduled follow-up visits to ensure efficiency.
Ensure that telehealth sessions begin as scheduled and are completed smoothly.
Greet patients, families, and visitors to introduce self and role.
Promptly answer calls from clinical teams and use approved methods of communication.
Assist in interpretation through approved methods as needed.
Protect patient privacy by adhering to information security policies and HIPAA guidelines.
Escalate and report patient safety concerns to the clinical team leadership.
Maintain accurate records of interventions and safety events.
Set priorities, problem-solve, and use sound judgment in difficult situations.
Exhibit sufficient oral and writing skills to communicate effectively.
Maintain attendance and punctuality records in accordance with department policy.
Keep a safe, clean, and functional environment, reporting any issues to management.
Experience Requirements :
Minimum three years of direct patient care experience, including creating telehealth policies.
Experience with quality, outcomes, and data collection.
Proven ability to analyze key legislative and organizational policies impacting telehealth.
Ability to identify data elements for quality improvement.
Development of efficient clinical telehealth workflows.
Experience drafting protocols to support telehealth initiatives.
Experience developing effective telehealth program strategies.
Proven ability to measure and communicate program successes.
Skilled in recognizing changes in patients’ clinical status that require escalation.
Experience identifying patient behaviors that pose safety risks.
Demonstrated success in collaborating with an interdisciplinary team.
Familiarity with telehealth or remote monitoring technology.
Experience with electronic medical records.
Past success identifying benefits and barriers to implementing telehealth.
Experience communicating telehealth concepts to organizational members and healthcare consumers.
Proven ability to recognize elements of a successful telehealth encounter.
Ability to consider language, culture, and disability in utilizing telehealth technologies.
