LPN or RN – Wound Care Nurse

Job Title : RN or LPN Wound Care Nurse

Location : ASVH Flagstaff

Schedule :

Flexible 8-hour shifts MondayFriday (no weekends) : 7 AM3 PM 8 AM4 PM or 8 AM5 PM

Position Summary :

We are seeking one skilled RN or LPN to provide comprehensive wound care management including assessments treatment and education for residents. The nurse will serve as a resource to the care team ensure proper documentation and maintain up-to-date knowledge in wound ostomy and continence care.

Key Responsibilities :

Perform wound assessments reassessments and develop individualized management plans.

Develop and implement skin care programs to prevent breakdown.

Revise care plans based on resident response and evaluate effectiveness.

Assess and manage pain; educate residents and families on pain management.

Provide staff in-services on wound prevention wound care ostomy care and incontinence.

Communicate effectively with nursing staff and physicians.

Consult with interdisciplinary team members to address resident needs.

Maintain accurate documentation in line with standards and policies.

Participate in training and continuing education to stay current with wound care best practices.

Requirements :

Current Arizona RN or LPN license.

Certification : CWOCN (Wound Ostomy Continence) or CWCN (Certified Wound Care Nurse).

Ability to obtain / retain an Arizona Fingerprint Clearance Card.

Current CPR / BLS certification.

Preferred : Experience in skilled nursing / long-term care.
