[job_card.job_description]Provide, coordinate, and direct home nursing care as per physician's orders and agency policies Responsible to the Clinical Manager Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses Conduct home visits with periodic office meetings Willing to travel up to 75 miles from the home clinic Compensation for mileage between patient homes Support between 5-10 patients per day with roughly 6-8 points each Handle types of cases such as Start of Care, Wound Care, and Follow-up Visits Complete all documentation within 24 hours of the patient appointment Schedule is M-F with rotations every 4-6 weeks for weekends On-call duty required once a week Regional coverage within specified metro area Productivity expectation of 30 units per week No guaranteed hours; opportunity to float to nearby clinics if necessary
