[job_card.job_description]Responsible for the efficient and accurate coordination of home health visits for patients Maintain professional, open communication with clinicians, patients, and caregivers regarding visit schedules Coordinate closely with intake, clinical management, & billing teams to ensure smooth transitions & accurate visit tracking Maintain compliance with Medicare and agency-specific visit documentation and frequency rules Monitor & report scheduling metrics & participate in quality initiatives to enhance efficiency and customer satisfaction. Th HH Scheduler position is an in-office position Detail-oriented, with strong organizational and time management skills Ability to work independently and as part of a team in a fast-paced environment Strong knowledge of Microsoft 365, including Outlook, Word and Excel. Ability to communicate effectively in writing and verbally Possess solid problem-solving skills and critical thinking skills High school diploma or equivalent required Experience in healthcare scheduling; 1-2 years Home Health scheduling experience preferred Working knowledge of Wellsky Kinnser preferred
