[job_card.job_description]Administer medications and treatments, as prescribed by healthcare professionals; Monitor and record vital signs and health indicators; Conduct regular health assessments and report and concerns or changes in condition; Implement and follow individualized care plans and health protocols; Assist with transportation to / from medical appointments, as needed; Maintain accurate and up-to-date health records and documentation; Report any incidents, accidents and unusual occurrences promptly; Participate in care planning meetings and provide input on health-related issues; Ensures that the residence has a “home-like” atmosphere that conveys the feeling that this is the individual’s home. Ensures the individual’s rights are upheld, that they are given responsibilities within the home and that they are active in making decisions about their home; Serves as an active member of each person’s interdisciplinary team, to include involvement in Individual Habilitation Plan development ensuring that all individuals are provided a program that develops independent living skills and meets the goals according to each individual’s ISP; Act as liaison, advocate and coordinator for individual’s involvement in support groups, self-advocacy, vocational programs, transportation, community activities, recreation and learning independent living skills; Follows all agency policies and procedures; Trains, monitors, and assists individuals, as necessary in daily living skills; Attends staff meetings, conferences, seminars and training relevant to the position; Accompanies individuals to and from appointments and activities as directed by the Support Manager; Builds a support network between the individuals and community members; Works with families / guardians to ensure quality services are being delivered that are consistent with MAS’ Policies and Procedures; Advocates for the individuals supported in MAS residences; Follows / carries out the Recreation and Leisure Calendar which outlines the activities interests scheduled for each individual on a daily basis; Accurately completes all “In-House” documentation including UIR documentation; Complete required documentation in the Electronic Health Record (EHR) System; Administers, monitor and record (signs MAR sheets) medication administration according to policy; Reports any errors as necessary following agency protocols and UIR procedures; Using Crisis Intervention Techniques as trained and appropriate; Trains, monitors, and assists individuals toward learning specific ISP goals; Successfully completes all required residential trainings as well as CPR, First Aid, Abuse & Neglect, Medication and Shifting Expectations : Changes in Perception, Life Experience, and Services; Assumes the responsibility of the “In-Charge” position as assigned; Works a flexible, full-time schedule, not less than 37 hours and not to exceed 40 hours per week in conjunction with the 3 : 1 staffing ratio; Completes all duties as delegated by the Support Manager and any additional duties by the nature of the position. High School Diploma (or equivalent); Bachelor’s degree preferred; Valid, active Licensed Practical Nurse (LPN) Certification with no restrictions; Valid Auto Driver’s License with a driving record in good standing; NJ Criminal Background and CARI Clearance; Intermediate to Advanced PC skills with proficiency in : MS Office including MS Word, Excel, Outlook and PowerPoint; Electronic Health Record software; Experience in community nursing and intellectual / developmental disabilities; Lifting or assist in lifting disabled adults as medically necessary; Provision of on-call support as determined by the program needs; Ability to maintain a high level of confidentiality; Must have high initiative, integrity, strong work ethic, good communication and analytical skills; Excellent time management, organizational skills and enjoy working in a fast paced environment;

INTRODUCTION
The LPN will be responsible for providing medical care and supporting the day-to-day activities of individuals in the Midland Adult Services (MAS) Residential DDD funded Program. The LPN will work in a team environment to ensure our clients receive the highest level of quality services, including administering medications, monitoring health conditions, assisting with daily activities and promoting a safe and supportive environment.
