Behavioral Health Connections (BHC) in Dunwoody, GA is seeking a dynamic and talented LPN Referral Management Weekday Varied shift Full Time10 hours Shifts that will review assessment referrals and work closely with the RN and other clinicians, to determine the appropriate placement of patients, seeking treatment. Assist the call center specialist with other duties, to enhance referral managment process.

Job Duties / Responsibilities :

• Reveiw assessment referrals received and collaborate with RN to assess patient needs.
• Assist with coordination of patient care
• Maintain patient recoreds, updating databases, and ensuring accurate documentation of patient information

Qualifications

EDUCATION

Nursing Degree, preferred Bachelors or Masters

LICENSURE / CERTIFICATION

Licensed LPN in current State or compact license

Current CPR, if required by a customer hospital. ("Customer hospital" means the hospital to which BHC is contracted to provide mobile assessment services.)

EXPERIENCE

Preferred 1-3 years related work experience in Behavioral Health
