Responsibilities
- Conduct in-home evaluations and assessments to determine patients' needs and develop comprehensive treatment plans
- Implement prescribed physical therapy interventions utilizing effective techniques and modalities
- Provide education to patients and their families on proper techniques and exercises for promoting mobility and functional independence
- Monitor patient progress and revise treatment plans as necessary to achieve optimal outcomes
- Document patient evaluations, treatment plans, and progress notes accurately and timely
- Collaborate with interdisciplinary team members to ensure coordinated and comprehensive care
- Adhere to ethical and professional standards, as well as compliance with all regulatory requirements
