Position Details :

Department : Discharge Case Management

Location : 123 Summer St, Worcester, MA 01608 (Saint Vincent's Hospital)

Hours : Full time, 40 hours weekly. Monday through Friday, hour options 8 : 00AM - 4 : 30PM, 8 : 30AM - 5 : 00PM, 9 : 00AM - 5 : 30PM

Primary Responsibilities :

Provides comprehensive case management and care coordination for a diverse panel of patients with identified needs spanning age, condition, and population variables

"Coaches" (rather than "teaches") patients to improve their health behavior to attain health-related goals, while working as part of a health care team (Providers, Nurses, Social Workers, Patients, Family, Caregivers, etc.)

Patient education focus designed in conjunction with Clinical Pharmacists to work with patients to optimize medication protocols and transitions

Work Closely with Clinical Pharmacist to manage HTN / DM disease and medications

Assist with medication adjustments by protocol

Perform medication teaching in collaboration with Clinical Pharmacist

Required Qualifications :

Graduate from an accredited school of nursing

Active, unrestricted licensure as a Registered Nurse in Massachusetts

Current Massachusetts driver's license with the ability to travel to nearby hospitals

Preferred Qualifications :

Certification in Geriatric Nursing or Geriatric Case Management

5+ years of nursing experience, preferably with older patients

Experience with hospital discharge case management

Proficient in computer use, the Internet and health information technology

Proven ability to work effectively both independently as well as a member of an interdisciplinary team

Demonstrated excellent communication, interpersonal and organizational skills, with a flexible and creative approach to problem solving

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
