Key Responsibilities
Telephonically manage comprehensive cases and coordinate care per established guidelines
Conduct holistic assessments addressing various patient needs and develop collaborative treatment plans
Advocate for patients by facilitating quality care delivery and implementing cost management strategies
Required Qualifications, Training, and Education
Graduation from an accredited college with a degree in Social Work required
License in Social Work preferred
Minimum 2 years of acute healthcare experience, including 2 years of case management experience
Basic computer literacy and proficiency in Microsoft Office applications
Knowledge of medical claims and coding systems such as ICD-10, CPT, and HCPCS
