Overview :

The Certified Nurse Aide (CNA) performs patient care activities and other tasks to assist the primary care, therapy, and adult day health team members.

Thome PACE serves Jackson, Lenawee, and Hillsdale counties providing participants with a wide range of medical and supportive services in our Day Center located in Jackson, Michigan or in the comfort of their own homes.

What you'll do :

• Assists in providing and / or maintaining optimal physical and emotional comforts to participants.
• Assists participants toward achieving maximum self-reliance and independence.
• Communicates with participants, families, staff, and the staff of other agencies.
• Participates in interdisciplinary team meetings.
• Provides basic ADL nursing care.
• Documents treatments / procedures, observations, nursing care, and participant activities.
• Reports changes in the participant's condition to the pertinent interdisciplinary team member.
• Demonstrates active participation in QI processes.
• Participates in the process of teaching and learning with others.
