Patient Care Coordinator - Care Management at Providence St. Joseph Hospital Orange in Orange, CA. This position is Full- time and will work 8-hour, Day shifts.

Under the direction of the Supervisor or Manager, this position is responsible for providing post discharge appointments for at risk patients discharging from the hospital. This position works closely as a healthcare team member to assist our hospital in providing excellent care and communication to our patients.

Required Qualifications :

• Coursework / Training and Completion of Medical Terminology coursework. Or Working knowledge of Medical Terminology.
• 1 year of Healthcare experience.
