Description

• Conduct evaluations of patients in both inpatient and outpatient environments, crafting personalized treatment plans tailored to individual needs
• Implement physical therapy techniques aimed at enhancing mobility, strength, balance, and overall function
• Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to achieve outstanding outcomes
• Foster education for patients and their families on therapeutic objectives, discharge strategies, and home exercise regimens
• Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with facility policies and standards
• Participate actively in interdisciplinary case rounds and conferences to ensure cohesive and coordinated care

Education

• Doctorate in Physical Therapy (DPT) or a relevant degree from a recognized program

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications (e.g., neurological, geriatric) are advantageous but not essential
