Internal Intake LVN

Primary Responsibilities :

• Review and process new referrals including order and clinical documentation.
• Build and maintain relationships with referral sources.
• Handle inbound inquiries from clients family members and field team members.
• Manage patient care coordination with field teams.
• Oversee resource coordination including specialty pharmacy DME supplies and lab arrangements.
• Provide hands-on care for troubleshooting as needed.
• Coordinate with sales acceptance of patients.
• Manage documentation and participate in the on-call rotation with clinical leaders.
• Collect and review client needs to support scheduling.
• Deliver exceptional client service for internal and external stakeholders.
• Promote continuity and safety in client care adhering to quality standards.

Experience and Education Requirements :

Minimum Skills and Abilities :

Work Environment : This role involves working in a high-paced office setting with continuous phone interaction. Physical requirements include sitting and standing for long periods occasional bending and stooping and lifting up to 10 lbs.
