This care is based on nursing theory and research to a designated group of patients and significant others. Delivers patient care by incorporating the principles of Relationship Centered Care. Uses the nursing process based on theory, research, evidence-based practice, and nursing standards. Assesses and evaluates patient care needs while applying critical thinking skills in patient management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with interdisciplinary healthcare teams to develop, implement, and evaluate the plan of care. Prioritizes patient care aspects including teaching, rounding, coaching, planning for post-hospital care, and delegating when appropriate. Communicates effectively and professionally with patients, families, and all members of the healthcare team. Manages assignments within the Care Delivery Model and demonstrates effective allocation of resources. Models the principles of Relationship Centered Care through various patient interaction actions. Considers patients' values, preferences, cultural diversity, expressed needs, and knowledge in care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to understand patients' perceptions of care. Initiates service recovery for patients and family members with concerns and escalates as needed. Practices safe and efficient patient-centered care while identifying barriers and opportunities for improvement.
