Now Hiring : FULL-TIME Physical Therapist (PT)- Home Health

Locations : Northern Nevada Including Reno, Sparks, Carson City, and Surrounding Areas

Make a difference where it matters most - at home We're looking for a dedicated Full- Time Physical Therapist (PT) to join our compassionate home health team. Help patients regain mobility, independence, and quality of life - all from the comfort of their own homes.

Key Responsibilities :

  • Provide skilled physical therapy services in patients' homes according to the plan of care.
  • Perform evaluations, develop and revise treatment plans, and set measurable goals.
  • Educate patients and families on exercises and self-care to improve mobility and safety.
  • Conduct home safety assessments and recommend modifications as needed.
  • Collaborate closely with physicians, nurses, and other team members to ensure quality outcomes.
  • Maintain accurate, timely documentation for each visit.
  • Participate in case conferences and communicate patient progress to the care team.
  • Support the patient and family in understanding goals, expectations, and progress.

    • Requirements :

  • Current Physical Therapist (PT) license in Nevada (required)
  • Minimum 1 year of home health physical therapy experience (preferred)
  • Strong clinical, communication, and problem-solving skills.
  • Ability to work independently and manage a flexible caseload.
  • Valid driver's license and reliable transportation.

    Compensation : $50.00 - $70.00 per hour
