Licensed Practical Nurse (LPN / LVN) – Night Shift
Facility : Ladera Center
Area : Albuquerque, NM
Class : LPN
Grade : 15
Shift : 2nd Shift – 7 : 00 PM to 7 : 30 AM (12-hour shifts)
Position Summary
Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides safe, efficient, and compassionate nursing care designed to promote positive patient outcomes and satisfaction. The LPN functions within the scope of practice defined by the State Nurse Practice Act , contributes to care planning and implementation, delivers direct nursing care, and supervises patient care performed by unlicensed staff.
Key Responsibilities 1. Patient Evaluation
Collects, documents, and reports objective and subjective patient data.
Observes patients closely and promptly reports any changes in condition to the RN.
2. Care Planning
Contributes to the development of individualized patient care goals.
Assists in creating and implementing nursing interventions.
Evaluates the effectiveness of interventions to support optimal outcomes and reduce re-hospitalizations.
Participates in the ongoing review and revision of patient care plans.
3. Direct Patient Care
Administers medications and performs treatments as ordered by physicians.
Documents care thoroughly and accurately using PointClickCare (PCC) .
Communicates essential patient data to RNs and physicians in a timely manner.
Assists physicians during rounds and patient examinations.
Participates in Utilization Management or Interdisciplinary Care Management meetings to coordinate discharge planning.
Educates patients and families regarding care, treatment, and discharge instructions.
4. Supervision of Unlicensed Staff
Assigns and oversees tasks according to the State Nurse Practice Act , Genesis policies, and staff competencies.
Communicates patient care updates and instructions during shift handoffs.
Monitors performance of unlicensed personnel and ensures assigned duties are completed accurately and safely.
Provides feedback and follows up on care delivery to ensure compliance with policies and standards.
5. Additional Responsibilities
Performs narcotic counts at the beginning and end of each shift.
Ensures clear, consistent shift-to-shift communication and documentation.
Promotes collaboration with other departments to ensure timely, individualized patient care.
Supports a respectful, team-oriented, and patient-focused work environment.
Participates in new staff orientation and in-service education.
Maintains confidentiality and protects all Protected Health Information (PHI) .
Works beyond scheduled hours as needed to meet staffing and patient care requirements.
Promotes and upholds a culture of safety and compliance with all regulatory standards.
Performs other related duties as assigned.
Compliance & Ethics
Complies with all federal, state, and facility regulations including HIPAA , the Federal False Claims Act , and Genesis Healthcare’s Code of Conduct .
Participates in all required training, audits, and compliance programs.
Reports any potential compliance concerns or incidents promptly.
Supports quality assurance, risk management, and process improvement initiatives.
Qualifications
Graduate of an approved school of practical or vocational nursing ( Required ).
Current LPN / LVN license in the State of New Mexico ( Required ).
Current BLS / CPR certification ( Required ).
Experience : Preferably in rehabilitative or geriatric nursing .
Ability to read, write, and communicate effectively in English to ensure patient safety and clear documentation.
Must provide verification of tuberculin skin test (TST) per state requirements and company policy (TSTs may be administered at the facility if needed).
Work Environment
Skilled Nursing or Long-Term Care Facility
Night shift, 12-hour schedule (7 : 00 PM – 7 : 30 AM)
Frequent standing, walking, and lifting required.
Exposure to infectious materials and conditions common to healthcare settings.
